You will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005.

About the Teams
Integrated Discharge Team
Barnet & Chase Farm Hospital
37 hours per week | Permanent
You will work within a busy social care team at Barnet Hospital, carrying out assessments to support safe and timely discharges. You will collaborate closely with the hospital Discharge Team and care providers to deliver effective, person-centred care in line with Hertfordshire’s Connected Lives principles.This fast-paced role requires strong organisation, the ability to manage competing priorities, and confidence working with complex and vulnerable individuals. Using sound judgement and a solution-focused approach, you will help patients return home quickly with appropriate support, promoting independence and wellbeing.
In this post you will be required to work 7 days 1 in every 8 weeks.

Transfer of Care Team - Lister Hospital
Stevenage
37 hours per week | Fixed Term
You will work as part of a busy Transfer of Care Team, completing social care assessments to ensure that people are discharged in a safe and timely manner. You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support. You will develop close working relationships with hospital staff and care providers to ensure effective care planning.
Older People West - Prevention of Admissions
Hemel Hempstead
37 hours per week | Permanent and Fixed Term to April 2027
The Prevention of Admissions Team works alongside NHS community Teams providing the social care aspect to crisis situations including carer breakdown, and ultimately Prevention of Hospital Admission. The teams aim to keep service users in the community to ensure they remain independent and do not become institutionalised by avoidable hospital admissions. This post requires travel in the south and west of the county.
Older People West - Area Team West
Hemel Hempstead
37 hours per week | Fixed Term to May 2027
The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.
Watford General Hospital Integrated Discharge Team
37 hours per week | Permanent
The Integrated Discharge Team at Watford General Hospital support a 7 day discharge service. This is a 37 hour a week position, and 1 in every 8 weekends you would be expected to work a Saturday and Sunday.
Our work is fast paced as we focus on supporting people to leave hospital as quickly and safely as possible, with a person-centred approach, with the majority of the work being through Discharge to Assess.
The usual hours are 9am – 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role.
About you
Essential criteria
- The team is community based and you must have a full UK driving licence and regular access to a vehicle for working purposes
- Experience of working in social care
