About the team

The Review Team East is a dedicated and experienced team committed to supporting adults to achieve the best possible outcomes while ensuring care and support remains person-centred, proportionate, and responsive to changing needs. We work closely with individuals, families, providers, and partner agencies to undertake annual reviews in a wide range of settings across the East of the county.

Our focus is on promoting independence, maximising strengths, and ensuring that commissioned services continue to deliver positive outcomes. We take a collaborative and strengths-based approach, building positive relationships with carers, families, and professionals to ensure people continue to receive the right support for them.

About the role

Although based in the review team the Carers Practitioner will have a dedicated focus on supporting unpaid carers. This role is specifically responsible for holding completing Carer's Assessments and reviews under the Care Act 2014. Working directly with carers, you will identify their strengths, needs and desired outcomes, ensuring they receive appropriate information, advice, support and access to services that help them maintain their wellbeing and continue in their caring role where they choose to do so. The role involves close partnership working with families, social care colleagues, health professionals and community organisations to promote carers' rights and ensure their voices are heard.
